Psych NP
Psychiatric care by telehealth, across Missouri

Medication is a tool, not the plan.

Psych NP is a psychiatric practice for children, teens, and adults. We evaluate carefully, prescribe conservatively, and teach skills you can use, with the goal that one day you need less from us, not more.

01

Skills, every visit

Appointments here are more than a med check. Every visit includes short, practical therapy: tangible skills to take home and use before we meet again.

02

The whole person

Sleep, habits, stress, and physical health all shape mental health. We look at all of it, and we reach for the most natural approach that can work.

03

Less, when possible

Medication can be what makes progress possible. We use it carefully, aim for the fewest medications that do the job, and help you come down when you are ready.

“I don’t believe in just throwing meds at problems.”

An Arabic word for light: a lamp that guides.

Dr. Elkomy brought the word to the practice as a symbol of hope and guidance, and it describes what both providers are after: care that helps you restore hope, function better day to day, and reach your fullest potential. Not just fewer symptoms; a way forward you can see, where you are heard, valued, and part of every decision.

You will always know who you are talking to.

Julie Dotzler

PMHNP-BC · Children and young adults · ADHD, anxiety, and depression · Addiction recovery

Julie became a nurse because of a nurse. When her six-year-old son was going through cancer treatment, an oncology nurse practitioner carried the family through it with skill, compassion, and hope, and reminded them that healing is about far more than medicine. Julie decided she could be that person for someone else.

For the next decade she cared for premature infants, critically ill children, and families facing the unimaginable, in pediatric intensive care, neonatal care, and emergency rooms. A position she took as a placeholder in addiction treatment became ten more years and a calling: it was there she trained as a psychiatric nurse practitioner. Across more than twenty years in healthcare, one lesson held, that meaningful connection is not separate from good care; it is part of it.

Julie sees children, adolescents, and young adults for ADHD, anxiety, and depression, and she works alongside people rebuilding their lives after addiction. She offers full ADHD evaluation and treatment, thorough and individual, never one-size-fits-all. She wanted a practice where no one is rushed and listening matters as much as diagnosing. Trust is huge, she says. We don’t judge.

Her father used to ask one question that still guides her: If not me, who? It is why she built Psych NP to be a place where people who have spent years feeling invisible finally feel seen, heard, and hopeful again.

Dr. Fatten Elkomy

DNP, PMHNP-BC · All ages, 4 to 94 · Geriatric psychiatry · Complex conditions

Dr. Elkomy first trained as a physician at the University of Alexandria in Egypt. When life changed course after her husband’s passing, she turned to nursing to carry her family, and found the work she now calls her passion.

She went on to earn a master’s in health administration, a master’s and a doctorate in nursing practice, and board certification as a psychiatric mental health nurse practitioner, and was recognized with an Excellence Award from the University of Missouri. She has practiced psychiatry since 2006 across every age, with particular depth in geriatric psychiatry and dementia care, inpatient and outpatient.

She is also a teacher and researcher: a preceptor for graduate nursing students, a published author, and a presenter at national and international conferences, including the American Academy of Child and Adolescent Psychiatry and the American Psychiatric Association, on trauma, culturally responsive care, and children’s mental health. In her practice’s annual surveys, 94% of her patients rated their care as excellent, a result she calls very, very rare in mental health.

She practices in partnership: your story and her professional judgment, working toward goals you set together. I'm just not interested in just reducing the symptoms, she says. I want to take people a step up in their life.
